What is collected, and what is done with it.

Written plainly, because a page nobody can read is not a disclosure. This covers the organizational health assessment and the contact form, which are the only two places this site collects anything.

What is collected

Through the organizational health assessment: your name, email address, phone number, organization name, your role, your answers to the assessment questions, the scores those answers produce, and the date and time you submitted them. Your consent state is recorded alongside them, including whether you agreed separately to be called or texted.

Through the contact form: your name, email address, organization, role, and whatever you write in the message field.

In both cases a coarse record of the network connection is kept for a short period, used only to stop automated abuse of the forms.

How it is used

Your assessment results are emailed to the address you provide so you have a copy you can forward. The results themselves are used to prepare the review conversation if you ask for one.

Your email address is used to send you the results and a short series of follow up messages about the assessment, which stop as soon as you book a review or ask to be removed.

Your phone number is used for one purpose, which is scheduling and holding the review conversation. If you did not check the separate box agreeing to calls or texts, it is not used for outreach.

Anonymized scores contribute to the benchmark comparison set. Names, contact details, and organization names are never part of that set, and no individual organization is identifiable in it.

What is not done with it

Your details are not sold, rented, or traded. They are not shared with other organizations for their own marketing. No advertising or tracking pixels are placed on this site.

How long it is kept

Assessment records and contact details are kept for twenty four months from your last interaction, so a retake can be compared against your earlier result. After that they are deleted.

If you ask to be removed before then, your contact details and answers are deleted within thirty days. Anonymized scores already contributing to the benchmark cannot be traced back to you and remain in the comparison set.

How to have it removed

Reply to any message you receive, or write to the address below and ask. No explanation is needed and nothing is asked in return.

Where it is held

Records are held in a managed database with access limited to the practice. Nothing is stored in your browser except your own in progress answers, which stay on your device so you can resume the assessment.
